Kahn Health PC ("Kahn Health," "we," "us") is a direct primary care practice located at 100 Valley Road, Suite 102, Montclair, NJ 07042. It applies to website visitors and people who respond to our ads. It does not replace our HIPAA Notice of Privacy Practices, which governs how we handle the protected health information of our patients and is provided to patients separately.
