Pricing Plans

*Please note, there is a one-time enrollment fee of $99

Individuals ages 65+: $150/month

Individuals ages 22-64:‍ ‍$130/month

Children ages 2-21: $85/month

Infants 0-6 months:
We do not see infants 0-6 months for routine DPC membership.
For specialized care for your infant, however, see our 4th Trimester Program!
